Here you will be able to update the details of the contact, track contact and support as well as adding notes and tasks.
The first section reflects the initial information added, including any comments and the details of the person making the contact if not the subject.
The second section takes the details of the first call we make to the family. All boxes need to be completed.
If a decision other than Two or Three Calls Needed is made as the outcome then the contact will be closed. If Two or Three Calls Needed is chosen then a new set of fields will appear that will enable you to record ongoing information. The contact will be listed in the list of open contacts as ongoing.
At the same time, if we hold a mobile number for the subject of the contact, they will be texted a link to complete a Warwick-Edinburgh evaluation regarding their wellbeing.
You can continue to add to the summary and leave the status as Open to record new support. Once you come to the end of the calls you can either select Closed or Referred as the status. This will remove it from the open list.
If the contact becomes a full referral, either after the first call or a subsequent call, you can select the referral from the drop down list.
This will connect the two and you'll see the link icon alongside the contact, this will take you directly to the referral.
Further down the form you'll find the space to add notes to this initial contact and add and assign tasks to yourself or colleagues.
